In response, the Centers for Medicare & Medicaid Services (CMS) issued its 2026 Physician Fee Schedule Final Rule, permanently allowing virtual direct supervision for Level 2 diagnostic exams — a change that took effect January 1, 2026. Law firm Reed Smith noted the shift is expected to expand access to imaging services, particularly in areas most affected by staffing shortages.

As ContrastConnect's guide explains, imaging center revenue is significantly impacted by cancelled contrast exams, among the highest-value services in the industry, due to a lack of supervision staff. Virtual supervision directly addresses this issue by enabling radiologists to supervise a procedure regardless of where it is taking place. The model also eliminates travel requirements, saving time that would otherwise be spent on long commutes, improving the availability of supervisory personnel, and better utilizing shifts by enabling the radiologist to supervise procedures taking place in multiple locations. As a result, imaging facilities can increase scan volumes and extend operating hours, ultimately leading to higher patient throughput.
